I'll try to answer but may not be 100% correct.

So for AV DVDs to sell on store shelves or be broadcasted in paid TV channels, all private parts need to be covered by mosaic. Japan’s AV industry started with “R” (restricted) movies. The movie has story, the actors really act, and the private parts mosaic covered. These movies can be viewed by people above 14. With the AV flourish and evolve, this tradition continues. The mainstream adult videos have story and the actors really act. Some of the AV stars, like Yuna Shiina and Akiho Yoshizawa, actually have the beauty, skill, and intelligence to become a regular movie star.
